Food Pantry – If you have a caseworker at Department of Social Services or if you have a caseworker at Goodwill, or the Charlotte Housing Authority, or Veterans’ Services, or Urban Ministries or Crisis Assistance Ministry, or another human services agency. they can call in a referral. If none of the above applies, please call Loaves and Fishes at 704-523-4333. Referrals to the Transformational Housing Program can be made at any time. Families may visit the Jackson Park Ministry Clothing Closet up to one time every 90-days. |

The referring agency must call Loaves & Fishes on your behalf to make the referral. | Information needed in order to make a referral: Name, Birthdate, Social Security Number and Gender for all household members. Address and Phone Number of head of the household. Employment information for any members of the household. |
