All new uninsured patients come in through an application process. Come to our University location at 8401 Medical Plaza Drive, Suite 300, Monday through Thursday between 8:30 am and 3:30 pm to apply in person. No appointment is needed to fill out the application and no paperwork is required for this initial visit. Once your initial application is completed, we will set up an appointment with our Intake Team to determine where you fall on our sliding fee scale. You will be given information regarding what to bring to your next appointment.
